First, they looked at factors that affect the prevalence of HIV-1 infection (whether the men were infected at the time of recruitment). Second, they measured the incidence of HIV-1 infection (new infections detected during the follow-up period). At the time of recruitment 20.7% of the men were HIV-positive. Men with a positive blood test for past herpes were five times more likely to be HIV-positive at the time recruitment. other factors significantly associated with being HIV-positive were self-reported genital ulcers in the past year and being widowers. Rates of HIV-1 infection at the time of recruitment were the same in circumcised men and uncircumcised men (20.71% versus 20.76%). when adjusted for other factors, there was no significant association between circumcision status and HIV-1 prevalence.

The second half of the study involved following 731 participants who started off as HIV-negative and who made at least one follow-up visit. Becoming HIV-positive during the follow-up period was independently associated with a positive blood test for herpes at the beginning of the study, and 18 times more likely in men who developed a first-time HSV-2 (herpes) infection during the follow-up period. during the follow-up period uncircumcised men developed 23 infections in 5686 months of patient follow-up (4.04 per 1000 months), while circumcised men developed four infections in 817 months of follow-up (4.89 per 1000 months). this means that uncircumcised men had a slightly lower (but probably not statistically significant) risk of HIV infection. when adjusted for other factors, circumcision status made no difference to the risk of infection with HIV.

A significant weakness of the study is its reliance on self-reported circumcision status – something about which men are surprisingly uncertain. while the investigators assumed that all the HIV infections were transmitted sexually, it is also possible (even likely) that men with genital herpes would seek medical care, and in doing so placed themselves at greater risk of HIV infection through non-sterile medical treatment (iatrogenic infection) – notoriously common in the over-stretched health services of underdeveloped countries.

This is yet another study that fails to confirm the “60% reduction in risk of HIV infection” claimed for circumcision in the three famous randomized clinical trials. I am a 25 year old male in excellent physical health. Recently I have been having sex with a girl, no oral for me (she did perform oral), just the basic stuff. I have only had sex with her 4 times, twice protected and twice unprotected. these were spread out over about 5 weeks. after having unprotected sex with her the first time (about 3 1/2 weeks ago) we didn't have sex for about 2 weeks. after having unprotected sex with her for the second time (about a week and a half ago) she called me two days later and said that she had a swab test and was positive for HSV2. She thought that it was me that had given it to her. She told me that about midway between the time we had unprotected sex both times (about 2 1/2 weeks ago) she had a bump and got tested. She said that it was just one and doctor told her it was probably a very mild outbreak. I immediately went to the doctor and got a blood test which gave the results of about 4 (out of 1) positive for HSV1 and .02 (out of 1) negative for HSV2. I have never had any symptoms, either cold sores or itching/burning or anything. Here are my questions:

1. My doctor told me that swab tests cannot differentiate between HSV1 and HSV2, is this correct? the information that I found online seem to suggest otherwise.

2. if she does have HSV2, what are the chances that I got it from her in the 4 times I had sex with her?

3. What are the chances that I do have HSV2 is my blood test results showed me negative as a .02 out of 1 required for a positive?

4. is there a chance that she doesn't have it and the swab test was inaccurate? I told her she needed to get a blood test to confirm.

5. if you generally shed the virus during an outbreak, and her one bump healed a few days before we had unprotected sex the second time, how long was she shedding?

6. if my blood test came back negative with .02 out of 1, does this amount increase over time, and if so, would that mean that I could still have it and just have a false negative?

I have been researching online extensively, but haven't found any answers really to these questions. Chlamydia is the country’s most prevalent infection, with over 1.2 million cases reported annually. according to 2009 data, the disease most greatly impacted females aged 15 to 29, but males of a similar age range were also heavily affected that year. Left untreated, Chlamydia can cause permanent reproductive system damage and chronic pelvic pain in women. People with undiagnosed Chlamydia infection are also at an increased risk for contracting additional STDs upon exposure.

It’s impossible to talk about STD awareness without highlighting genital herpes, a disease affecting more than 50 million Americans to date. Though many assume that signs for STDs like herpes are easy to pinpoint, in reality, the majority of people infected with genital herpes will never experience outbreaks. Though herpes disease isn’t known to be fatal, it can produce recurring symptoms and put those infected at risk for other STDs, including HIV. Females are statistically almost twice as likely to contract genital herpes as their male counterparts, but as part of its STD awareness campaign, the CDC emphasizes the fact that all sexually active individuals are at risk for herpes infection. It’s important to note that genital herpes is transmitted through skin-to-skin contact so it can be transmitted during sex play. Sexual intercourse is not required to contract herpes.

HIV is country’s most feared STD, as it can attack the immune system and eventually progress to AIDS. according to CDC studies, men who have sex with other men account for over half of all new U.S. HIV infections each year. Females, on the other hand, represent about 27 percent of annual new HIV infections, the majority of which are transmitted via heterosexual contact. Despite HIV’s ability to weaken the immune system, those infected will often remain asymptomatic for months or even years following infection. With HIV, AIDS prevention and immune system preservation are the primary goals, and while there is currently no cure for HIV, antiretroviral medications can halt the progression toward AIDS and help those infected manage and maintain their health.

The Importance of STD Testing

Since signs for STDs are by no means a given, all sexually active individuals are advised to undergo regular testing to confirm or rule out the presence of infection. As part of its STD awareness campaign, the CDC publishes testing guidelines for various at-risk groups. All sexually active men and women are advised to screen annually for common infections such as Chlamydia, HIV, genital herpes, and gonorrhea. Yearly HPV testing is recommended for women, and those with an increased risk of syphilis (primarily men who have sex with men) are told to screen annually as well. by undergoing STD testing as per the CDC’s recommendations and upholding safe sexual practices, individuals at risk for STDs can help stop the spread of infection and protect themselves simultaneously.
